What you will do:

* Conduct Care Act assessments and develop tailored support plans for adults aged 18+ with physical disabilities and those over 65.

* Collaborate closely with partner agencies to ensure coordinated and comprehensive care.

* Handle adult safeguarding processes with confidence and diligence.

* Perform Mental Capacity Assessments and engage in relevant court work, including Court of Protection applications.

* Apply your solid understanding of relevant legislation to guide your practice and decision-making.

* Communicate effectively with clients, families, and team members while contributing positively to our team dynamic.
