What you will do:

* Conduct assessments for clients in the community to identify their needs and develop appropriate care plans.

* Implement tailored interventions to enable clients to achieve maximum independence.

* Collaborate with other healthcare professionals to ensure comprehensive care delivery.

* Maintain accurate records and documentation as required by the Health Care Professions Council.

* Travel between sites and client homes, utilizing your driving skills to cover the community area effectively.

To be successful in this role, you will need a Degree or Diploma in Occupational Therapy and must be registered with the Health Care Professions Council. Previous experience working at Band 6 level is essential, ideally in a community setting, though acute experience is also desirable. A valid driving licence is preferable due to the nature of this community role.
