What you will do:

* Conduct comprehensive assessments of needs and risks, including safeguarding investigations under Section 42 of the Care Act 2014.

* Collaborate with service users and carers to develop and implement personalised care and support plans.

* Coordinate care by working closely with partners from health, housing, and the voluntary sector.

* Respond to safeguarding concerns accurately and swiftly, taking appropriate protective actions when needed.

* Support individuals in decision-making, applying the Mental Capacity Act when necessary to ensure their best interests.

* Maintain clear, accurate case records and fulfil all statutory duties with diligence.

* Participate in multi-agency meetings and case conferences to advocate for service users.

* Promote a person-centred, strengths-based approach to practice, enabling increased independence and resilience.
