What you will do:
- Conduct comprehensive assessments for adults with learning disabilities, identifying risks and safeguarding needs.
- Develop and implement tailored care plans that promote independence and respect service users' dignity.
- Manage and prioritise a complex caseload efficiently, maintaining accurate records and documentation.
- Collaborate with health professionals, statutory bodies, and families to ensure coordinated service delivery.
- Participate in multidisciplinary meetings, discharge planning, and safeguarding processes.
- Identify and respond to safeguarding concerns according to local and national policies.
- Prepare for and engage in supervision and team meetings, contributing to the development of colleagues.
- Share knowledge to contribute to service evaluation and promote best practice in social work.
- Be flexible, working across different locations and participating in duty rotas.
