Key Responsibilities

Carry out comprehensive assessments and create tailored care plans for individuals with learning disabilities.

Provide high-quality support and advocacy, ensuring client needs and rights are upheld.

Collaborate with families, carers, and other professionals to ensure a holistic approach to care.

Monitor and review care plans regularly, adapting them to meet clients' evolving requirements.

Person Specification

Qualified and registered Social Worker with relevant experience in learning disabilities.

Strong assessment and care planning capabilities.

Excellent interpersonal and communication skills.

Ability to work autonomously as well as part of a multidisciplinary team.

Location & Working Environment

Based in Gateshead with only 2 days per week required on-site.

Office situated conveniently within the borough, with access to local amenities and transport links.
