Job Description
Health Care Co-Ordinator
35 hours per week (working hours can be flexible)
Rate £17 – £21 per hour depending on experience
The Lantern Community provides supported living for just over 50 adults with diverse learning abilities (Companions) in a unique and outstanding rural setting.
We are currently looking for an experienced Health Care Co-ordinator with a nursing background. Please note due to our registration with the CQC, the post holder will not work as a nurse in our setting but will assist the companions and staff around health-related matters.
Overview of the role:
The Health Care Co-ordinator will ensure that effective health care plans are in place and facilitate care staff in houses in implementing these health care plans.
They will promote holistic well-being and provide training in health-related issues to staff and those with diverse learning abilities
They will assist people we support with access to health-related services and facilitate a more effective working partnership with the NHS and primary care services.
Qualifications and experience:
Five years experience providing health care and nursing
A relevant professional qualification and nursing experience
Excellent interpersonal and communication skills
Proven IT skills to create Easy Read material and create health related passports and other documents
Experience working with individuals with diverse learning abilities
Enthusiastic and organized with excellent communication skills and good use of initiative.
Ability to establish effective working relationships both within the Community, and with outside agencies, groups and organizations
Committed to delivering a quality, person-centred service
Key Responsibilities:
Create effective health care plans with people we support and their care team
Help those we support to access health care, especially where those with diverse learning abilities face difficulties accessing the health care they need.
To act as an advocate and challenge health inequalities on behalf of those whom we support
Creating or providing access to Easy Read material about relevant health issues and health care plans for those with diverse learning abilities
Providing training and education on health-related issues to people we support and their support& care teams
Supervise teams in houses in health-related practices
Create health related passports together with a person we support and their supporters in an area where they would benefit from a passport, such as meal support, incontinence support etc.
To supervise and motivate all companions and support staff around health matters.
To work closely with the Healthy Living Team Leader and other health professionals to create a holistic approach in health-related matters
Maintain a high level of knowledge, keeping abreast of new trends within the local and national health care system and any other local health care related services.
Open minded and interested to learn at least the basic elements of the philosophies, methods and values that guide The Lantern Community via Anthroposophy and applied in Social Therapy.
