An exciting opportunity has arisen for a Band 6 Liaison and Diversion Practitioner - Mental Health Nurse in Harrogate or Scarborough, UK , where you can bring your expertise in Mental Health Nursing, Social Work, or Occupational Therapy to a team that truly values your skills. Tees, Esk & Wear Valleys NHS Foundation Trust is searching for a talented individual to join the Liaison and Diversion NY&Y team. This ongoing role offers flexible working schedules, full-time or part-time options, with competitive hourly rates: £26 on weekdays, £34 on Saturdays, and £41 on Sundays.

What you will do:
- Conduct assessments with individuals referred through the Liaison and Diversion service to understand their needs.
- Develop and implement care plans in collaboration with multidisciplinary teams.
- Provide support and interventions tailored to the needs of individuals with mental health, learning disability, or substance misuse issues.
- Liaise effectively with police, courts, and other services to ensure coordinated care for individuals.
- Maintain accurate records and ensure compliance with all relevant policies and procedures.
