What you will do:
* Assessment and Care Planning: Conduct statutory assessments under the Children Act 1989 and develop Child in Need, Child Protection, or Looked After Children plans, ensuring the best outcomes for children.
* Support for Children and Families: Provide both practical and emotional support to families; assist them in accessing essential services like respite care, equipment, and financial aid.
* Multi-Agency Working: Liaise with health, education, and voluntary sector professionals and contribute to Education, Health and Care Plans (EHCPs) to build supportive networks around each child.
* Safeguarding: Identify and manage risks of abuse or neglect, and carry out Section 47 child protection investigations as needed to ensure children's safety.
* Transition Planning: Support young people with disabilities as they transition to adult services, ensuring continuity of care by collaborating with adult social care teams.
* Advocacy and Rights: Promote the rights and well-being of disabled children, ensuring compliance with the Equality Act and SEND Code of Practice while advocating for their best interests.
