We have an exciting opportunity for a Primary Care Nurse who has experience in Long Term Conditions to join our integrated Primary Care team at HMP Askham Grange and HMP New Hall.
HMP Askham Grange is an open category D female prison based in Askham Richard, near York, housing around 120 women . You will be joining an established and experienced team, supporting women aged 18 plus, the site also includes a mother and baby unit.
HMP New Hall is a female adult and young offenders institution offering medium level of security with a population of 344 females.
We are looking for a primary care nurse who has experience in Long Term Conditions.
Hours:
- Monday-Friday - 37.5 hours per week
- 7am - 5pm
- 4 days/week at HMP Askham Grange
- 1 day at HMP New Hall
- Ad-hoc weekends. - 1 weekend in 4
This position is full time, you'll receive an annual salary up to £33232 - £40211per annum FTE - depending on experience.

Candidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
What you'll be doing
As a Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
- Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Qualifications required:
- Registered nurse - Adult
- Experience in Long Term Conditions
- Evidence of Continued Professional Development
- Understanding of the complexities of multi-disciplinary and multi-agency working
- A non-judgmental and compassionate approach
- Excellent communication skills
