What you will do:

* Conduct Care Act Assessments, Mental Capacity assessments, and facilitate Best Interest outcomes.

* Complete Community Deprivation of Liberty assessments and engage in Continuing Healthcare (CHC) checklists and Decision Support Tool (DST) processes.

* Operate from a Strengths-Based perspective, focusing on Least Restrictive Practice.

* Support the team by triaging referrals and prioritising day-to-day case management activities.

* Collaborate confidently with professional partners within a multidisciplinary team (MDT) approach.
