What you will do:

* Conduct comprehensive community assessments to support clients following hospital discharge or to prevent re-admission.

* Maintain accurate records of client progress and treatment outcomes.

* Offer guidance and support to family members and caregivers to facilitate ongoing rehabilitation at home.

Requirements:

* HCPC registration required.

* Minimum of 2 years of experience working as an Occupational Therapist.

* Car driving is preferable to effectively reach clients across the region.
