What you will do:
- Conduct thorough assessments to understand patients' specific needs and devise personalised care plans
- Provide rehabilitation support and interventions to maximise patients' independence
- Work closely with multidisciplinary teams to ensure seamless patient care and transitions
- Continuously evaluate and adjust treatment plans to reflect patients' progress towards goals
- Keep comprehensive records of patient assessments and progress notes
- Educate patients and their families about care plans and rehabilitation strategies

Requirements of the Occupational Therapist:
* Degree or equivalent in Occupational Therapy
* Recent UK experience working as an Occupational Therapist
* Recent NHS experience
* Registered with the HCPC
