What you will do:
- Conduct comprehensive assessments of patients within their homes and care settings
- Develop and implement tailored treatment plans to promote patient independence and quality of life
- Prescribe and fit essential equipment, ensuring patient safety and comfort
- Identify and address falls risks, delivering appropriate interventions to reduce occurrences
- Collaborate with multidisciplinary teams to deliver holistic care solutions
- Maintain accurate records and report on patient progress
- Travel between patient homes and care homes in the Lewisham area, bringing necessary equipment
To be successful in this role, you must have at least 1 year of experience working within the community, particularly with older adults. Experience in prescribing and fitting equipment, together with a strong understanding of falls risks and interventions, is essential. You will need to be a car owner and driver to efficiently navigate your duties, carrying equipment as necessary. A valid HCPC registration is also required.
