What you will do:

- Provide a comprehensive clinical pharmacy service to inpatient wards at the City Hospital Campus.

- Conduct medicines reconciliation to ensure patient safety and accuracy in medication records.

- Offer patient counselling to enhance understanding and adherence to medication regimes.

- Prepare TTOs (To Take Out) ensuring patients have the correct medication upon discharge.

- Deliver specialised medicines advice to the wider multidisciplinary team.

- Collaborate with Medicines Management Technicians to provide optimal care in medication management.
