Job Overview:

As a key player in our Adults Hospital Team, you'll be pivotal in enhancing the lives of adults during their hospital journey and their transition beyond. You'll engage with dedicated hospital staff, multidisciplinary teams, and external agencies to secure safe discharges, carry out assessments, and ensure individuals receive appropriate support when moving on from hospital care.

What you will do:

* Conduct assessments and reviews for adults in hospital settings, focusing on discharge planning and recognising support needs.

* Develop and review care plans using person-centred approaches that support best practice standards.

* Safeguard vulnerable adults by managing risks and ensuring timely, effective interventions.

* Collaborate with multidisciplinary teams, aiding in the recovery and discharge process.

* Offer advice and support to patients and families on care options and services.

* Coordinate with external agencies for smooth transitions from hospital to home or other settings.

* Comply with legislation and policy requirements while maintaining accurate records.

* Participate in supervision, team updates, and training to foster professional growth and service delivery enhancement.

Essential Qualifications and Experience:

* A recognised social work qualification (CQSW, DipSW, or equivalent) is required.

* Registration with Social Work England (SWE) is essential.

* Significant experience in adult social work, especially within hospital or health-related environments.

* Strong legislative knowledge, including the Care Act 2014 and Safeguarding Adults.

* Excellent communication skills and the ability to work within multidisciplinary teams.

* Competence in managing complex cases in fast-paced settings.
