We have a thrilling opportunity for a Social Worker within Adults in Lambeth UK, earning £32 per hour on an ongoing basis. This is an exceptional prospect to work full-time within a dynamic team, contributing to a home care mobilisation project. Your role will involve assisting service users in transferring their care to neighbourhood providers, ensuring that both service users and their families are comfortable and informed about these changes. Start ASAP and enjoy a hybrid working arrangement: you'll be in the office Monday to Friday for the first four weeks, and then twice a week thereafter.

What you will do:

- Conduct thorough reviews of care needs for adults, tailoring plans to meet individual requirements.

- Engage with service users and their families to discuss changes in care providers, offering reassurance and support.

- Document and report on assessments, care plans, and follow-up activities to ensure compliance and high standards of social work.

- Stay updated on local resources and services to better advise and support service users.

Requirements of the Social Worker:

* Degree or equivalent in social work

* Current Social Work England registration

* Significant frontline experience in Social Services within the UK working as a qualified Social Worker
