What you will do:

* Conduct comprehensive assessments of adults in need, ensuring a person-centred approach in line with local and national policies.

* Develop and implement care plans to address identified needs, collaborating with individuals, families, and other professionals to ensure effective service delivery.

* Work effectively within a multi-agency setting, participating in safeguarding investigations and providing expert knowledge and intervention when required.

* Maintain accurate and up-to-date records, preparing detailed reports and case notes that reflect assessments, decisions, and care plans.

* Undertake risk assessments and reviews, ensuring any concerns are promptly addressed, and interventions are tailored to individual circumstances.

* Facilitate and attend meetings, reviews, and case conferences, advocating on behalf of service users while continuing to promote independence and dignity.

* Stay informed about changes in legislative and policy frameworks, adapting your practice to comply with these changes.
