Social Worker G7 Discharge to Assess Team - Wolverhampton - £33.20 Hourly - 3 months, with potential extension

This is an exhilarating opportunity for a Social Worker to join the Discharge to Assess Team in Wolverhampton. You will be earning a rewarding £33.20 per hour in a full-time role that promises variety and challenge. This temporary position is initially for three months, but there is a potential extension for the right candidate. What you will do:

- Conduct D2A discharges in Pathway 1, Pathway 2, and Pathway 3 settings.

- Complete safeguarding enquiries to ensure the wellbeing of individuals.

- Respond to urgent and unplanned community visits promptly.

- Cover Saturday duties once every five weeks, with subsequent Monday off.

- Work closely with the Duty team, spending three days a week in the office when needed.

- Be available for urgently required duty visits in the hospital or community during home working days.
