What you will do:

- Conduct Care Act assessments of needs, focusing on extra care sheltered accommodation voids.

- Perform carers' assessments and reviews to ensure holistic support for individuals and families.

- Complete scheduled reviews and facilitate safeguarding enquiries to maintain client safety.

- Present cases at panels, including joint funded panels, to secure necessary resources and support.

- Collaborate with the legal team, prepare statements, and attend court hearings when required.

- Participate in the duty system, represent the service at GP MDTs, and actively contribute to team meetings.
