What you will do:

- Conduct assessments and reviews to ensure individuals' needs are effectively met, focusing on promoting independence and well-being.

- Identify and facilitate access to reablement opportunities and relevant services.

- Apply a strength-based approach to help individuals assess their own needs and access community resources.

- Monitor ongoing service provision and ensure effective coordination to maintain focus on outcomes and well-being.

- Provide professional advice and support to individuals, carers, and partner organisations.

- Perform duties under the Mental Capacity Act and safeguarding responsibilities as a Safeguarding Officer.

- Develop skills by maintaining a social work caseload with appropriate supervision.

- Contribute to health and social care assessments, including continuing healthcare assessments, once trained.
