What you will do:

* Conduct comprehensive assessments and develop support plans for adults aged 18+, ensuring each plan is tailored to individual needs and focused on desired outcomes.

* Perform Mental Capacity Act assessments and facilitate best interest decisions to support residents' rights and choices.

* Respond to safeguarding concerns promptly and effectively, contributing to investigations and ensuring adherence to legislation and best practices.

* Collaborate as part of a multi-disciplinary team, working alongside health and social care professionals to provide cohesive, person-centred support.

* Maintain meticulous and timely records, respecting confidentiality and data protection laws at all times.

* Participate in our duty rota, ensuring that urgent cases and referrals are managed efficiently.

* Engage in reflective practice to continually improve your professional expertise and maintain high service standards.

* Adhere to all relevant legal frameworks, including the Care Act 2014 and the Mental Capacity Act 2005.

* Work flexibly across our diverse neighbourhood teams, including Heywood, Middleton, Pennine, Rochdale North, and Rochdale South.
