What You Will Do:

* Collaborate with Multi-Disciplinary Teams: Work closely with healthcare professionals and external agencies to deliver integrated and person-centred care.

* Ensure Legal Compliance: Adhere to relevant legislation, including the Care Act 2014, Mental Capacity Act, and other regulatory frameworks.

* Promote Independence: Advocate for patients by empowering them to maintain their independence and improve their quality of life through effective support planning.

Skills and Qualifications:

* Essential: Degree in Social Work with current Social Work England registration.

* Experience: Previous experience working in a hospital discharge team or adult social care setting.

* Knowledge: Strong understanding of the Care Act 2014, safeguarding procedures, and best practices in hospital discharge.

* Skills: Excellent communication, analytical, and report-writing skills, with the ability to manage complex caseloads.
