Band 6 Nurse - Community Mental Health Team

Location: Worcester

Salary: £27.00 per hour

Hours: Monday - Friday, 9:00 am - 5:00 pm (Hours negotiable for the right candidate)

What you will do:

Clinical Duties:

- Provide comprehensive mental health assessments for service users with complex and enduring mental health conditions

- Develop, implement, and evaluate individualised care plans in collaboration with service users and their families

- Utilise evidence-based interventions including psychoeducation, crisis intervention, and relapse prevention strategies

- Manage a caseload of service users, ensuring timely and appropriate care is delivered

- Conduct risk assessments and develop risk management plans to uphold safeguarding principles

- Offer specialist mental health advice and support to service users, carers, and other professionals

- Administer and monitor medication effectiveness in line with prescribed treatment plans

Teamwork & Professional Development:

- Collaborate closely with a multi-disciplinary team, including psychiatrists, psychologists, and occupational therapists

- Participate actively in case discussions, care reviews, and clinical meetings

- Mentor and support junior staff, students, and support workers

- Engage in continuous professional development, staying up-to-date with the latest mental health care practices

- Adhere to relevant legislation and guidelines such as the Mental Health Act and NICE guidelines

Communication & Liaison:

- Build strong therapeutic relationships with service users and their families to promote recovery and independence

- Liaise effectively with GPs, social services, and external agencies to ensure coordinated care

- Maintain accurate and up-to-date documentation using electronic patient records

Essential Requirements:

- Registered Mental Health Nurse (RMN) with valid NMC registration

- Experience in a CMHT or similar community mental health setting at Band 5 or above

- Strong understanding of mental health conditions, risk assessment, and crisis intervention

- Excellent communication and interpersonal skills

- Ability to work both independently and as part of a team

- Knowledge of relevant legislation affecting mental health services

- Proficiency with electronic patient record systems

- Full UK driving licence and access to a vehicle for community travel

Desirable Criteria:

- Additional qualifications in CBT, DBT, psychosocial interventions, or non-medical prescribing

- Experience supervising junior staff or students

- Previous involvement in service improvement initiatives
