What you will do:
* Deliver safe, patient-centred, effective, and evidence-based occupational therapy services in community environments.
* Engage with an adult service to provide acute and long-term therapy and clinical activities.
* Conduct holistic assessments, develop and implement care plans for patients with complex needs, especially among the frail elderly cohort.
* Employ clinical reasoning skills to assess, plan, implement and evaluate patient interventions, focusing on physical, psychological, social, leisure, and employment needs.
* Promote high quality and harm-free care using an evidence-based practice approach.
* Collaborate with an integrated multi-disciplinary team including nurses, physiotherapists, paramedics, and other allied healthcare professionals.
To be successful, you need HCPC registration and a valid driving licence.
