Social Worker- Adults within Community Team in Tavistock, Devon earning £31 - 35 Hourly . What you will do:
- Undertake Care Act Assessments to assess and support the needs of adults in the community.
- Carry out Mental Capacity assessments and Best Interest outcomes, ensuring decisions are made in the best interests of the individuals involved.
- Complete Deprivation of Liberty Safeguards (DoLS) assessments with proficiency.
- Navigate Continuing Healthcare (CHC) checklists and Decision Support Tools (DSTs) as part of your assessment processes.
- Work from a Strengths-Based perspective and demonstrate Least Restrictive Practice in all interventions.
- Support the team duty function by triaging referrals and prioritising your daily case management activities effectively.
- Collaborate confidently with professional partners in a multidisciplinary team (MDT) approach.
