What you will do:

* Carry out Care Act assessments and strengths-based support planning, optimising available resources.

* Conduct carers assessments and capacity assessments with subsequent Best Interest decision-making processes.

* Engage in Continuing Health Care assessments and compelling interdisciplinary and multi-agency working collaborations.

* Lead safeguarding investigations and support individuals transitioning into alternative accommodations.

* Involve yourself in crucial resettlement processes, advocating for out-of-area placement returns to the local scene.

* Prepare detailed reports including Ministry of Justice Tribunal reports and Social Circumstances reports for Mental Health Tribunals.

* Equip yourself for vital Hospital Discharges and Community Treatment Reviews. Handle Court of Protection applications with precision.

Qualifications and Experience:

* A qualified Social Worker with a proven track record working with adults with learning disabilities in a statutory role.

* Preferred experience with liquid logic systems is advantageous.
