What you will do:

* Conduct and complete assessments and reviews, tailoring support plans to the needs of individuals with learning disabilities and their carers.

* Work collaboratively in a multi-disciplinary team to deliver a comprehensive and integrated approach to social care.

* Prepare thorough, needs-led assessments and reassessments, ensuring all care plans are up-to-date and effective.

* Engage with community settings to foster strong working relationships and ensure resources are optimised effectively.

* Undertake Best Interest and Mental Capacity Assessments, dealing adeptly with safeguarding issues.

* Optionally oversee non-qualified staff and students, sharing your expertise to guide upcoming professionals.

* Occasionally work outside standard hours to cater to urgent client needs.

Qualifications:

* Social work qualification recognised by Social Work England

* Current Social Work England (SWE) registration

* Evidence of continuing professional development

Experience:

* Completing assessments and reviews

* Working with individuals and their carers in a community setting

* Preparing needs-led assessments/reassessments and support plans

* Using IT systems effectively

* Dealing with safeguarding issues

* Undertaking and completing Best Interest Assessments and Mental Capacity Assessments

* Working in multi-disciplinary teams within a Social Care or Health setting

* Experienced post holders with a minimum of 3 years' experience.
