What you will do:

* Assess and plan tailored treatment programmes that meet the individual needs of patients in recovery

* Collaborate closely with multidisciplinary teams to ensure well-rounded patient care

* Monitor and evaluate patient progress, adjusting therapy plans as necessary to achieve optimal outcomes

* Provide education and support to patients and their families to facilitate ongoing recovery

* Maintain detailed records of patient assessments, interventions, and outcomes in line with professional standards

Requirements:

* Applicants must be registered with the HCPC and have at least two years of experience in the field.

* A valid driving licence is required, as the role may involve travel between locations within Norwich.

* You should be committed to working within the Professional Body Code of Conduct/Practice, maintaining a professional standard of care, and following all relevant legal and safety guidelines, including the Health and Safety at Work Act and Infection Control training.
