Calling all Occupational Therapists ready for a new challenge in Community Mental Health! Join our dynamic team in Rochdale, located at Stansfield Place, Newbold, for up to 37.5 hours per week on a temporary contract running from January 2025 until the start of March 2025. Work hours are from 09:00 to 17:00, Monday to Friday, perfect for those who like their weekends free.

What you will do:
- Conduct assessments to determine the need for longer-term support under the Care Act 2014 and Care Programme Approach.
- Collaborate within a multi-disciplinary team to assess, plan, and implement brief, individualised care programmes.
- Assess occupational therapy needs and plan therapeutic interventions using the Model of Human Occupation, ensuring accurate record-keeping with up-to-date reports.
- Use standardised assessment tools like OSA, OCAIRS, and MOHOST to ensure outcome measures are evidenced, promoting partnership with relevant stakeholders.
- Develop partnerships with agencies, services, and individuals to support patients' optimal care.
- Engage in risk assessment and management formulation, fostering patient-focused interventions that respect social, cultural, and spiritual needs.
