Locum Social Worker - Hospital Team in Wolverhampton
Are you ready for an exciting opportunity to excel in your career as an Experienced Social Worker? We are seeking a versatile and motivated Social Worker to join our Hospital Team at New Cross Hospital in Wolverhampton for a locum position earning £33.20 per hour on a contingent labour contract. This full-time role, spanning 37 hours per week from Monday to Friday (09:00:AM - 05:00:PM) , is initially for a 3-month duration. Your expertise will be invaluable in supporting patient discharge processes and completing assessments under the Discharge to Assess model.

What you will do:
* Conduct comprehensive assessments in pathways 1, 2, and 3 under the Discharge to Assess model to ensure smooth patient transitions from hospital to community or care settings.
* Participate in multidisciplinary team meetings to coordinate care and develop patient-centred plans that promote their well-being and independence.
* Maintain accurate and up-to-date records and documentation, ensuring compliance with relevant policies and procedures.
* Provide advice, guidance, and support to patients and their families, ensuring they understand available options and resources.
* Contribute to the continuous improvement of service delivery by collaborating with colleagues and sharing best practices.
