What You Will Do:

* Collaborate with the Multi-Disciplinary Team (MDT) to plan, monitor, and evaluate work.

* Navigate and assess social care services under the Care Act 2014, supporting adults and carers.

* Provide guidance and advice for adults with acute or complex long-term needs.

* Conduct Best Interest assessments related to Deprivation of Liberties.

* Participate in safeguarding processes and multi-agency conferences.

* Communicate complex information, care and support planning, and address barriers to understanding.

* Submit funding applications and engage in community forums to promote positive social work.

* Mentor and support newly qualified social workers and other team members.
