What you will do:

* Assess and treat patients with a variety of physical impairments in the community

* Develop tailored rehabilitation programs to facilitate individual patient recovery

* Collaborate with a multidisciplinary team to ensure holistic patient care

* Provide thorough documentation and maintain patient records in line with HCPC standards

* Educate patients and their families about therapy procedures to enhance treatment effectiveness

* Drive to patient homes or community settings, making car travel an essential part of this role
