Occupational Therapist - Social Services Discharge Service

Pay Rate: £32 per hour

Location: Knowsley Street, Bury, Lancashire, BL9 0SW

Contract Type: Immediate start, ongoing

What you will do:

- Conduct comprehensive assessments of individuals' needs and deliver customised occupational therapy interventions to promote safe, independent living in their homes.

- Evaluate and adjust care packages to ensure they are suitably tailored to meet the specific needs of each patient.

- Implement single-handed care strategies, ensuring safety and compliance with best practices.

- Provide guidance on moving and handling techniques to maintain patient safety and assist carers in their roles.

- Work closely with multidisciplinary teams, including social services, health providers, and community organisations, to provide holistic care solutions.

- Maintain detailed and accurate records of all assessments, interventions, and outcomes in compliance with professional standards.

Skills and Requirements:

- Degree or equivalent qualification in Occupational Therapy.

- Current HCPC registration.

- Proven experience in single-handed care and moving/handling practices.

- Strong interpersonal and collaborative skills.

- Excellent organisational abilities to manage caseloads efficiently.

- Familiarity with relevant legislation and best practices in social care discharge planning.
