What you will do:

* Work across ICU, respiratory, cardiology, enhanced care, and surgical wards, applying your expertise to manage complex cases.

* Plan and facilitate efficient discharge processes for patients transitioning from ICU step-down care.

* Autonomously handle the rehabilitation of post-ICU patients, ensuring comprehensive care plans are implemented.

* Collaborate flexibly with your multidisciplinary colleagues to provide exceptional patient outcomes.

Requirements:

* HCPC registration.

* Experience in acute healthcare inpatient settings, with a strong focus on respiratory, surgical, cardiology, neuro, and neuro-rehab.

* Proven discharge planning capabilities.

* Desirable experience within an ICU setting.
