As a Community Psychiatric Nurse/Care Coordinator, you will play a vital role in providing holistic care and support to our service users. What you will do:
- Manage a personal caseload as part of the community team, ensuring professional accountability and responsibility.
- Coordinate care with other healthcare professionals, conduct regular reviews, and foster communication across multidisciplinary teams.
- Independently perform assessments, care planning, and coordination, demonstrating excellent interpersonal skills and cultural competency.
- Lead regular team meetings to discuss and evaluate client care, advocating for a flexible and inclusive approach.
- Address the physical health needs of service users with long-term conditions and escalate concerns as necessary.
- Empower service users to take responsibility for their care and support their adoption of recovery strategies.
- Handle risk assessment and management, safeguarding, and incidents efficiently and in line with Trust policies.
- Administer medication accurately and maintain complete and thorough electronic patient records.
Essential requirements:
- Registered Adult Nurse, Mental Health Nurse, or Learning Disabilities Nurse.
- Mentorship qualification.
- Demonstrable experience of research-based/reflective practice and clinical experience.
- Experience in interagency working and caseload management.
