What you will do:

* Assist qualified occupational therapists and physiotherapists during community visits.

* Promote patient independence and reablement by supporting therapy plans.

* Collaborate with team members to develop effective care strategies.

* Conduct assessments and provide regular feedback to the supervising therapist.

* Help devise personalised rehabilitation plans for clients.

* Maintain accurate and timely patient records.

Must be a car driver. Daily journeys to patients' homes in the Sefton area are essential for this role.
