Position Overview:

* Role: Band 6 Rehabilitation Occupational Therapist (Locum)

Key Responsibilities:

* Conduct assessments and deliver rehabilitation services for patients referred for planned Occupational Therapy intervention in their own homes.

* Develop and implement personalised care plans to improve patients' independence and enhance their quality of life.

* Collaborate with a multidisciplinary team to ensure comprehensive and holistic care delivery.

* Maintain accurate and up-to-date patient records and documentation to track patient progress and treatment outcomes.

* Educate patients and their families on therapeutic exercises and adaptive equipment usage.

* Participate in regular team meetings to review patient progress and manage cases effectively.
