Job Description
Applocum are currently recruiting for ANP/ACP's for a Liverpool based Home Visiting Service
System: Adastra (Training can be provided)
Home Visiting Sessions - *Driver is Provided*
As part of this service you will be completing home visits across the Locality, 30minutes is allocated for each appointment. This is for Adult patients only and is classed as an Acute Home Visiting Service however experience in Palliative care would be a great addition as generally patients are homebound so over 60 years of age.
Free parking at the base site where the shifts start.
Shift Patterns
7 Day a week service covering times when GP Practices are unable to accommodate Home Visits.
Monday – Friday shifts on a regular basis – 7pm – 11pm shifts
Saturdays and Sundays – 9am – 1pm, 10am – 3pm, 3pm – 6pm etc
