What you will do:

- Deliver strengths and assets-based social work services in line with the Care Act, Mental Capacity Act, and Mental Health Act.

- Collaborate closely with Herefordshire & Worcestershire Health and Care NHS Trust to deliver specialist mental health services.

- Use the Three Conversations approach to focus on clients' strengths, personal assets, and community resources.

- Conduct assessments, create support plans, provide interventions, and evaluate outcomes effectively.

- Handle safeguarding enquiries and work adeptly in complex situations using professional judgment and a range of interventions.
