What You Will Do:

Conduct Care Act (2014) assessments and reviews using both telephone and face-to-face methods.

Manage a caseload efficiently, ensuring timely recording of interventions to support effective case management.

Build and maintain strong, professional relationships with partner agencies, such as Health, Police, Housing, and the Private and Voluntary Sector (PVI).

Identify opportunities to support carers, promote independence, and enhance the wellbeing of service users.

Respond to urgent situations requiring duty cover, triaging and assessing incoming work for appropriate action.

Lead Section 42 Safeguarding efforts, chairing multi-agency meetings, and conducting Mental Capacity Act (2005) assessments.
