What you will do:

* Manage an allocated caseload, ensuring that all service users receive high-quality and personalised care.

* Complete referral forms and liaise with teams for smooth client transitions between services.

* Maintain accurate records and report any incidents in adherence with Trust Policy.

* Safeguard service users by following established procedures and raising alerts when necessary.

* Administer medication safely and according to legal and Trust guidelines.

* Provide leadership and supervision to junior staff, enhancing their skills and practices.

* Coordinate with multidisciplinary teams and external agencies to deliver comprehensive care.

* Engage in regular practice reflection and supervision, fostering a culture of continuous improvement.
