What you will do:
At Grade 7
Manage a restricted caseload under the guidance of a Team Manager or Senior Social Worker.
Conduct assessments, reviews, and reassessments using social work methods to drive change and improve life opportunities.
Work within legislation, policies, and procedures to assess and support adults and their circle of support, offering personal budgets where appropriate.
Maintain accurate and clear records in line with legal and policy guidelines.
Arrange and chair planning meetings, reviews, and ad hoc meetings as required.
Collaborate with partners and agencies to plan and coordinate cost-effective, quality services.
Undertake short-term interventions in the Home First service to prevent long-term social work involvement.
Engage in multi-agency forums, promoting effective collaboration and diversity awareness.
Participate in safeguarding enquiries, liaising with multi-agency partners and taking appropriate legal action.
At Grade 8

Collaborate with partners and agencies to assess, plan, and coordinate innovative, cost-effective services.
Supervise allocated non-qualified team members.
Undertake short-term interventions in the Home First service to prevent long-term social work involvement.
Engage in multi-agency forums to promote effective collaboration and inclusivity.
Participate in safeguarding enquiries, liaising with multi-agency partners and taking appropriate legal action.
Essential Requirements:
* Experience working with learning disabilities in statutory social work roles such as reviewing, casework, and care planning.
* Solid practice in social work with proof of continual professional development.
* Proven experience in safeguarding vulnerable adults.
* Registration with an appropriate professional body and relevant professional Social Work qualification.
