Position : Social Worker - Learning Disabilities Team

Location : Hackney, London

Hourly Rate : £32.00 per hour

Employment Type : Ongoing, full-time (36 hours per week)

Key Responsibilities :

* Conduct thorough assessments in accordance with the Care Act (2014) and the Mental Capacity Act (2005).

* Engage in Court of Protection work, advocating for the best interests of service users.

* Develop and coordinate personalised support plans for adults aged 18 and over, requiring ongoing care and reviews.

* Collaborate with a multi-disciplinary team, including Nurses, Psychologists, Psychiatrists, Occupational Therapists, Behavioural Analysts, and Speech and Language Therapists to deliver comprehensive care.

* Participate in multidisciplinary meetings and contribute to the continuous development of service provision.

* Ensure the safeguarding of vulnerable adults and maintain accurate, up-to-date records and documentation.
