Key Responsibilities:

* Conduct thorough assessments to address mental health and social care needs, ensuring timely hospital discharges.

* Apply your expertise in statutory duties to manage a complex caseload efficiently.

* Collaborate with carers and professionals to achieve the best outcomes for individuals.

* Perform social care needs assessments, Human Rights assessments, and mental capacity assessments.

* Act as a Safeguarding Enquiry Officer, working closely with the Safeguarding Adults Manager to ensure safe outcomes.

* Advocate for equality and the effective application of health and social care policies.

Candidate Requirements:

* Excellent communication and assessment skills.

* A passion for supporting carers and individuals in the mental health sector.

* Strong knowledge of adult social care and mental health legal frameworks.

* An Approved Mental Health Professional (AMHP) Qualification is highly desirable.
