What you will do:

* Work within the Multi-Agency Safeguarding Hub to coordinate care for vulnerable adults.

* Assess and implement a range of social care services, ensuring compliance with relevant legal frameworks and standards.

* Provide supervision and support to newly qualified social workers and Care Navigators within the service.

* Carry out complex needs assessments and create comprehensive support plans.

* Collaborate with multi-disciplinary teams and external services for effective signposting and referrals.

* Engage in safeguarding processes and ensure adherence to best practice and legal requirements.

* Conduct Best Interest assessments in relation to Deprivation of Liberties.

* Effectively communicate sensitive information to stakeholders and collaborate effectively with community forums.
