Key Responsibilities:

* Rehabilitation Plans: Develop and implement tailored rehabilitation plans for each patient, ensuring effective progress and care.

* Frailty Patient Care: Provide high-quality physiotherapy services to frailty patients within the inpatient setting, supporting their rehabilitation journey.

* Multidisciplinary Collaboration: Work alongside the multidisciplinary team to ensure comprehensive, coordinated patient care.

* Mentorship: Support and guide Band 5 Physiotherapists, sharing expertise and contributing to their professional development.

* Continuous Improvement: Incorporate the latest best practices, especially in frailty and rehabilitation services, to enhance patient care outcomes.

Requirements:

* HCPC registration as a Physiotherapist.

* Experience in frailty and rehabilitation services.

* Proven ability to work collaboratively within a multidisciplinary team.
