We have an exciting opportunity for a Long Terms Conditions Nurse to join our Primary Care team at HMP Thorn Cross, a Category D prison based in Warrington.

In this role you will have the opportunity to provide the highest standards of healthcare to our patients, without having to compromise on efficiency and quality.
This role is over 4 days.

Hours: Monday to Friday- 7am - 5pm or 7.30am to 5.30pm
Weekends and Bank Holidays- 7.15 am - 12noon

This position is one day a week , you'll receive an annual salary up to £47,466 per annum - depending on experience.

Candidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.

What you'll be doing

As a Long Term Conditions Nurse, your responsibilities will vary. You will

  • Be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings.
  • You will identify immediate health needs including, any external services or prescribing requirements that prisoners
    may have received e.g. prior to remand or repatriation onto a wing
  • A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc.
  • Provide proactive intervention to urgently address improvement recommendations from CQC and HMPP inspections .

Qualifications required
  • Registered Nurse- General with relevant NMC Registration
  • Advanced Nurse Prescriber/ Practitioner
  • Educated to degree level
  • Knowledge of health and social care models across the criminal justice system and of national and local initiatives relating to physical health
Although not essential ideal applicants would have experience in a hospital, community or prison environment.
