We are seeking a skilled and dynamic individual for the position of Care Co-Ordinator Band 6 within Community Services in Guernsey UK. This full-time role offers a salary between A4C BAND 6 £46152 and £55579 per annum, with a bar of £57375 to £62310 annually. What you will do:

* Provide comprehensive care coordination for an allocated caseload of adults with learning disabilities and/or autism

* Conduct thorough assessments of needs for adults and facilitate Carer Assessment when necessary

* Manage a complex caseload, working collaboratively with a team of professionals addressing complex care and support needs

* Act as the lead professional within the Community Learning Disability Team, ensuring coordinated support for adults with complex care management needs

* Report to and receive supervision from senior colleagues, fostering an environment of continuous professional development

* Build and maintain strong collaborative relationships with adults, their families, and extended support networks
