What you will do:

- Deliver comprehensive clinical pharmacy services on hospital wards, maximizing the quality of patient care.

- Provide essential dispensary cover when needed, ensuring efficiency and precision.

- Manage discharge medications, ensuring safe and seamless transitions for patients.

- Screen outpatient prescriptions to verify accuracy and safety, maintaining the highest standards.

- Conduct homecare prescription screening, ensuring all regulations are met and optimising patient outcomes.

- Collaborate closely with healthcare professionals to optimise and personalise patient care.

Requirements:

- Must hold registration with the General Pharmaceutical Council (GPhC).

- Availability to work Monday to Friday, 37.5 hours per week in full-time capacity.

- Demonstrated experience in a hospital pharmacy setting is required.

- Exceptional communication skills with a proven ability to thrive in a multidisciplinary team.
