Key Responsibilities:

Conduct assessments and develop tailored intervention plans for elderly patients with mental health needs.

Collaborate within a multidisciplinary team to deliver comprehensive care and support.

Implement therapeutic activities to improve patients' well-being and independence.

Educate and advise patients, families, and carers to promote positive health outcomes.

Ensure accurate and timely documentation to maintain high standards of care.

Preferred Experience and Requirements:

Experience in mental health, particularly with older adults, is advantageous.

Previous Band 6 experience is highly desirable.

HCPC registration is essential.

A valid driving licence is preferable to support client outreach.
